　　1、ATA单证册申请表。
　　2、申请人的身份证明文件。其中申请人为自然人的，提供身份证或护照复印件;申请人为企业法人的，提供企业法人营业执照的复印件;申请人为事业单位的，提供事业单位法人证书的复印件。

　　⑴申请人名称栏、地址栏中的英文应打印填写，并注明邮编。
　　⑵申请人为自然人的，身份证明文件指身份证或护照;申请人为法人的，身份证明文件指企业法人营业执照或者事业单位法人证书。
　　⑶授权代表是指代表申请人持ATA单证册办理国内外报关手续的人，可以是申请人的货运代理或报关代理。授权代表姓名的英文栏须打印填写。
　　⑷联系人指办理ATA单证册申请手续的人，可以是申请人的职员，也可以是申请人的报关代理或者货运代理。代理人须提供申请人的授权委托书。
